The La Candelaria Hospital will have nurses managing kidney transplant cases



The Nuestra Señora de La Candelaria University Hospital in Tenerife has incorporated the role of kidney transplant case manager nurse into its portfolio of services, in order to provide personalized follow-up to patients waiting for a compatible organ.

Currently, 40 users are being treated with the case manager’s action protocol, although the aim is to increase the number and carry out these protocols on the greatest number of patients who need it, given that there are more and more cases of people who require this intervention.

The figure of the nurse case manager provides greater coordination and global management between the services that care for the user, apart from facilitating the continuity of care in a coordinated and planned manner, as indicated this Friday in a statement by the hospital center, attached to the Ministry of Health.

Kidney transplants are performed at the Canary Islands University Hospital, but it is the Nuestra Señora de La Candelaria Hospital that cares for patients who require this type of intervention, and that is why it has launched this consultation to provide personalized treatment prior to receiving the transplant at the reference center.

With the nurse case manager, patients who are receiving hemodialysis treatment, while waiting for their compatible organ to arrive, can receive personalized follow-up treatment, receiving a monthly consultation where the clinical history is assessed and the continuity of care is ensured. patient and that their care plan is being followed.

In addition, they maintain close contact and fluid communication with the doctors who handle their case, since the nurse knows the patient’s specific situation and can identify those with special needs who may be susceptible to complications.

Among the studies performed on users are blood tests, consultations with services such as Urology, Digestive or Cardiology, among others, x-rays, electrocardiograms, CT scans, ultrasounds and specific assessments depending on the age or sex of the patient. .

In addition, the health center has had a liver transplant case manager since 2018, given that it is the reference hospital in this area of ​​the Archipelago.
